What is on the Confirm your information page?
The read-back of name, date of birth, refusals, state, and who to call.
Confirm your information is a read-back of what a paramedic or clinician reads off your page. No one will ask you to confirm it at the time, so it is worth a slow look now.
What is on it
Under What your scanned record shows:
- Name, Date of birth and Sex, as the page will show them
- Refuses: anatomical gift and vaccination, when you have chosen them
- Signed in: the state, and the date you signed, once there is one
- Who to call: the clinician or agent you named for the public page, with their relationship and phone number
Each row has a Change link to the screen where you edit it. The date of birth is corrected on Your details. Whether the page shows the year or the whole date is set on Choose what responders see (first set on the document walk).
Under Held, but not shown, the page lists the full date of birth when your page shows only the year. The full date stays with us and inside your signed document. If your page shows the whole date, anyone who scans your code sees your name and exact birth date together, and this page says so.
What continuing does
This is right, continue saves nothing. It takes you on to your emergency contacts. Everything shown here was entered on other screens.
Nothing here is final. You can change any of it later, though once you have filed a signed document, changing what it says means filing a new one.
What to look for
- Does the name match the ID in your wallet and the document you signed? A page a clinician cannot tie to the patient in front of them is a page they set aside.
- Does the state match the one printed on your document?
- Is the date of birth right, and is it shown the way you want?
